Okinoshima-cho is the administrative hub of the Oki Islands, an archipelago the Japanese Imperial court used as a place of political exile — Retired Emperor Go-Toba was sent here after losing the Jōkyū War in 1221, and the island still has the shrine and historical sites associated with that period. That exile history gives the island an unusual cultural depth for somewhere this remote. As the main settlement in the Oki group, it's the most livable of the island municipalities, with a ferry port, some services, and a real fishing community. Still very isolated, but the most viable option in the Oki Islands if you have a specific reason to be there.
